Which one? Silence of the Lambs is based off some real serial killers. The Orphan character has an illness that is a real life thing. And, well, the Visit could happen, though I don't know of any stories of such a thing happening in real life.

I'm thinking of The Orphan - the disease could occur in real life, so it's theoretically "realistic", but someone actually committing that kind of long-term deception and murder would probably be wildly implausible and unlikely

So, I mean, what does "realistic" mean

I think it would be easier to ask for examples of films (especially thrillers) you consider realistic.

Yes many do. I personally do believe in something beyond the physical world. But when I hear the word realistic when it comes to media, that to me simply means something without supernatural elements.
