Age regression isn’t my specific fetish, but in general, I think you can get some mileage out of the fetish-as-condition if you realistically treat it like a condition. If it’s something that has to be worked around or accommodated, it’s something the reader can empathize with. If you just want the sex without the pathos, though, I think voluntary is often better.
