For some reason, there are a gazillion Touhou doujinshi on E-Hentai with the "armpit sex" tag. I almost never see that tag on other doujinshi. Maybe it's a fandom in-joke?

Fandom in-joke is the correct guess. The main player character Reimu wears a dress with detached sleeves, exposing her armpits, thus "delicious miko armpits" is a running gag.
