As with most Night Vale fanart, the picture is 100% fanon. She never gets any visual description; she could easily look like your average non-creepy real-world human. All the canon creepiness has to do with her voice and her dialogue.

I have no idea, it could be either ^_^; The series involves so little physical description that you can draw basically anything and call it fanart, and you wouldn't be wrong.
