I think both of those cases are practicality reasons. As it is Rory spends hours in makeup and you would probably need a small bit of CGI to make it look like Dinklage is missing his nose. In the books for example Rorge (I think) has no nose at all, that's not in the show. Dagmer Cleftjaw's jaw is actually split down to the middle, in the show he has a small scar on the side of his face. Etc.

(That reminds me that the CA:FA has a behind the scenes thing about how they made Red Skull noseless. It was nifty.)
