Depending on where she's from it's plausible. Where I'm from it's pretty common to refer to other people that way without meaning anything by it. And, frankly, it sounds like she's taking you seriously and trying not to do it.

I know you [seem] suspicious because there have been a couple times she's done it but...well. An ingrained habit is a very hard thing to break and slip ups do happen when you're not thinking about it, and from what you've said it sounds like it's likely the case.
