Call up the insurance company and ask what the hell is going on. That is bizarre. Possibly a mix-up, possibly dishonest.

I did. They said that the Doctor who treated me was Out of Network (though she IS In-Network in a different location). The building I went to was In-Network but apparently she isn't. I don't know how that works.

They said the place where the bill came from is also Out of Network (because the name on their site does not match the name on the bill).

Told me to contact the medical provider which I will have to do tomorrow.

You should get billed for the place you personally went to, you're not responsible for every place the doctor may work at. Basically, it sounds like someone, maybe her, believes you saw her in a location that you didn't and billed you as such. It might be a mistake, if she works at different clinics and got confused about where she saw you in particular. The only advice I can think of is A) find that doctor and talk to her about this, and B) find out where your paperwork for that visit is, assuming you filled out forms and such, they should be on record at the place you visited.
