Keep looking on their site. Do you have this insurance through your job? I know we had a special agent that dealt with us and I could call him basically just to complain or have him push me through to someone. He only sold my company the insurance but he at least could give me numbers of who to call and in what department.

Best of luck and believe me it's a nightmare. I got lucky with my one test where it was supposed to be $2000 out of pocket (it's a highly specialized test that I luckily only need every 10 years) and was quoted by the insurance company that I would have to pay it. Three months after the test, I get an explanation of benefits where the insurance company paid for the whole thing.
