As a Belgian citizen I can say that that for sure is not a surname that is at all common here/sounds remotely like it would be Belgian in origin.

I know a dentist named Doctor De'ath, which I greatly appreciate. He's British AFAIK.

It's British, actually. Nothing much to do with Ath, except British people named Death at some decided "Death" was too morbid, but not morbid enough to just pick a new name. e_e

They may think it's Belgian.
