I think it was the bad timing that he fell into the Odinsleep right during his conversation with Loki.

I think he was also wrong to condemn Loki right when Loki was hanging from the abyss ("I could have done it, Father! I could have done it! For you! For all of us!" "No, Loki"), which is what caused Loki to let go and fall.
