Eh, a long, long, long time ago with FF7. I don't remember the details, but it was on some ancient forum and I was acting like a young 12 year old jackass (I think there was some flame war or whatever, but I *think* I took an April Fool's joke too far). To my knowledge, the site and forum no longer exist on the internet (the forum was an Inside the Web one, iirc) and it's also been a decade plus an odd few years since then, I was able to waltz back into FF7 fandom just recently with no issues whatsoever. Of course, I mostly fan about the game with one of my best friends and only them, so eh!

(In hindsight, I did deserve to get my ass kicked for it at the time, though)

Other than that, I've only really left fandoms because I've gotten fed up with the other fans.
