Yes, and it is nothing more than a fetish, or a severe case of body dysmorphia. We don't cater to anorexics who think they're fat, or the people who are convinced that they're hideous or that one or more of their limbs don't belong to them. But let some 45 year old man who's lived as a man, even fathering children announce he's really a woman, and we're all supposed to agree.

I wouldn't put it so harshly. I think if someone who's 45 and has fathered children still has gender identity issues at that point in his life, he's got a genuine and serious problem which is deserving of sympathy and help. But it is a problem, not a normal state for a person to find themselves in, and it is asking a bit much to expect the entire population to change the fundamental understanding of gender upon which so many of their own identities rest to accommodate a very small percentage who can't quite make it work for them.
