I've always read it as a face you'd make when you're like "Oh my god I can't believe this" or as tumblr would say, "*internal screaming*." There's a 90% chance I'm completely wrong though.

It always depends on the context. In what context have you seen this being used?

OMG I can see that now!! I got that it was an upside-down face, but I was thinking that it was like tilting your head when you're confused, except you're EXTRA confused so you tilt your head all the way around........but that didn't really make sense. Thanks! :D
