Things can get gross in the outer ridges of the ear, behind it, and around piercings if people leave them in all the time. It's not just the inside that needs maintenance. Usually if you get your head wet in the shower every day, toweling the ears off with the hair is an afterthought, but not everybody does that.

I have bits of hair condition stuck in my ear sometimes. Rarely but it happens.
