Because fiction does not exist in a isolation to reality. Fiction informs reality. A harmful image in fiction reinforces the idea that real life should be harmful.

A woman in a skimpy outfit on tv reinforces the social idea of women as objects. a black man committing a crime on tv reinforced the idea of black men as violent criminals. Something that is problematic in fiction supports real life problems. Thats why we call it problematic. If you can't see this then I think it's best that you be isolated from human contact.

If someone looks at a piece of media and goes "You know what? I should base my personal morals and views around this without conducting any research of my own on the actual world and how things operate, or using a bit of my own common sense on how not to be a shitty person", then they should not be allowed to consume media.
