I used to read a ton of fantasy until I got sick of that very trope. Read enough of it and you'll start to hunger for something better and richer.

Not all fantasy is epic fantasy, though. For that matter, not all epic fantasy is medievalesque settings with MacGuffin plots. Are there way too many Tolkien imitators who inexplicably keep getting published? Sure. But they're a drop in the bucket compared to all of the urban fantasy, detective fantasy, comic fantasy, gaslamp and other historical fantasy, fairy tale fantasy, weird fiction, magical realism, portal fantasy, etc., etc. that's out there.

Mostly the timeperiods it draws from. I have difficulty relating to anything older than the 1890s.

Interesting. I think part of what I like about the earlier time periods is that it does feel more removed from what I'm used to - but maybe that's just what I've come to expect in fantasy fic.
