In addition to everything other people mentioned already :

- The Black Company series by Glen Cook : no elves or dwarves, some monsters but uncanny ones, a lot of dark magic worked in a "realistic" or at least original way - no fireballs and the such. It starts as the chronicles of a company of mercenaries that happen to work for the evil sorcerer, who would probably be the bad guy in a usual fantasy story.

For a more old school but original tone, Gene Wolfe's "The Book of the New Sun" series, 4 books :

The Sword of the Lictor
The Citadel of the Autarch
