Asking for job references

I am applying to a job now that is asking for three references. Two are good to go, but for the third, I think I need to reach back to someone I interned for in 2012. I think we had a very good working relationship, and he has provided references for me in the past, but it's been so long now that it feels awkward just sending him an email out of the blue. Any advice for making the email not awkward?

Two years is not that long ago. Ask as you would any other person.

as the other anon said, 2 years isn't really that long. When I have to do something like that (usually for someone I haven't talked to in much longer than 2 years) I make a little intro spiel about how I've been doing in the last few years and how their guidance helped me get there, and segue into how I could really use their help again if at all possible.

Definitely ask. I graduated undegraduate in 2009 and graduate in 2010 and just recently (June) asked a professor from each to be my references for a licensing exam where you have to really put effort in, it's like a 5 page reference form to fill out, not just to be on call in case someone calls.

Just politely ask, no harm!

2012 is not that long ago, and the point of internships is for references. If you've used them as a reference before, chances are they have something written up for you already. I'd go for it. As for making it not awkward, I'd go for the general niceties: "hello, I don't know if you remember but it's me, I interned for you in 2012, etc." It would also help if in the e-mail you list a couple things you did as an intern just to refresh their memory - for places that go through a lot of people, that's very helpful.

Something people haven't mentioned: don't email if you think it's going to be awkward. Emails can be ignored or put off--call or go in person.
