Taking Tylenol for headaches even though I'm supposed to watch how much I take because of some other meds I'm on. My doc does blood tests and so far no liver damage. I should really just stop and ride out the headaches. :/

Can you ask a pharmacist for an alternative that won't interact with your current medications?

Assuming it's specifically NSAIDs that you have to stay away from: if they're really severe headaches, you can ask for a prescription for something like tramadol. Used sparingly, it's usually very good for short term pain management.

I'd kind of like to stay away from opiods, but thank you for the idea.

Tramadol isn't a typical opiod, which is why I recommended it. It's slow acting, which makes it have a lower propensity to cause dependency.

It's also a hell of a lot better than potentially damaging your liver just because you have a headache.

I'm not sure about the USA but it's still available through prescription here (last time I checked anyhow).

Caveat: While Tramadol was initially marketed as being lower risk for drug dependency*, it hasn't worked out that way. It is addictive and abused with regularity, especially by people who ended up taking it long term for chronic conditions. So unless those headaches are killer, I'd avoid it.


* People said the same thing about Oxycontin when it first hit the market, and I think we all know how that turned out.

Sure, it worked out that way because it was being prescribed for long term pain control, often for people with chronic pain conditions. ...And I mean... there's basically no real therapeutic dose of any opioid that isn't going to contribute to dependency when it's being prescribed long term.

It's still significantly lower risk than other opiods when it's used short term.

Uh, Tramadol most definitely can cause severe dependency, with the same sort of rate as most other opioids.

It's also generally contraindicated in those with liver and kidney problems due to the chemicals involved in its metabolism and the way the body then gets rid of the metabolites.

Please be more careful when giving out this sort of advice.

It can with long term usage, yes. Not short term intermittent usage. I'm not wrong about it being less likely to cause dependency than other opioids under those conditions.

And it's not contraindicated for medications likely to cause kidney or liver problems as long as it's being used correctly. The problem with labelled usage is that tramadol is usually prescribed for treatment of chronic pain, which is assumed to be long term use.

It's often one of the prescriptions handed out to liver patients post transplant, so.

Isn't basic Tylenol acetaminophen/paracetamol though? If that's what OP needs to stay away from (especially re: liver damage), then NSAIDs might actually be an option for them.

This. I was going to suggest ibuprofen. I find 200mg works better than 500mg acetaminophen, for me. And I get migraines.

Not necessarily. With some medications (and liver conditions) nearly any NSAIDs are contraindicated.

Acetaminophen isn't an NSAID though. Which is why people who cannot take NSAID's with meds take tylenol.

My bad, I meant to say that with some liver conditions basically every OTC analgesic is not recommended. I don't know what exactly is going on with OP, so I just assumed that might be the case.

Ah, in that case yeah, liver disease + tylenol or NSAIDs = probably best to consult a physician or a pharmacist.

True, but if OP is being told not to use the standard Tylenol then the active ingredient is Acetaminophen.

Acetaminophen is not an NSAID, it's an analgesic that works on the CNS with very little anti-inflammatory action, and it's potentially much more harmful in lower doses to the liver than an NSAID (which is more likely to cause kidney or GI problems) due to the way it's metabolized.

It'd help if OP clarified, but if they're being told not to use Tylenol then they're not being told to avoid NSAIDs. Whether NSAIDs themselves also cause problems for OP's condition/other medications obviously depend on what those are.

OP, the best thing you can do is explain your situation to a pharmacist, tell them what other meds you're taking/what your other issues are, and ask what your alternative options are.

Yeah, I was distracted by the stream. As above, I meant to say that in some conditions all OTC analgesics including NSAIDs are not recommended. I have no idea what is going on with OP so I just assumed that was probably the case if Tylenol was off the table.
