It's Rachel's last name too, since she's Jake's cousin. I don't think the others' names were ever revealed.

I remember that it was her father who was related to Jake's family because they were saying her mother didn't feel that comfortable around his family since the divorce, but I don't remember if it ever said if it was Jake's mom or dad who was related to her father. They could still have different last names, as far as I know.
