At least in this case, it's better to do it manually. The reason it's so rough is because I was trying to sculpt in a small area and around multiple protrusions, so I didn't have much room to get tools/fingers in to smooth down the surface. One of the protrusions has since been removed (it was preventing a hole that I'll need to thread something through later from getting sculpted over), so I can get in there with sandpaper now, but it's still a small enough area that I'd be worried about damaging the rest of them.
