I always thought Pterry was because of Pteppic from Pyramids, and how the P in there was a silent P and then the fans thought 'hey, let's do that to Terry as well' and then it spread to Neil because of Good Omens.

Maybe it was just because Pyramids had that P-before-t pattern in general. I do know that I have never heard it be associated with Wodehouse.

I believe 8 is referring to Psmith (http://en.wikipedia.org/wiki/Psmith). (Not that I've ever read a Psmith story; I've only read a couple of the Jeeves books and short stories.)
