Brother from another mother

So I'm confused. I used to assume "brother from another mother" referred to affairs (or, more innocently, half-brothers for other reasons). But lately I've heard it in the context of "we're like brothers." Which is it, or can it be both?

I've only ever used it for unrelated people who are close. Also, sister from another mister.

Wait, I've never heard "brother from another mother" meant literally. Has that ever been said, ever? Boggling my mind. I'm pretty sure as a slang phrase it's always meant close buddies.

I have, from people with half-siblings who don't realize its a phrase other people use.

I have, cause he's literally from another mother. So can be used both way, but I think usually just for close friends.

It can mean both. On a side note: The flipside gender version is a "Sister from another mister." Though I never hear people use this.

Technically it can mean both, but the "close friends" interpretation is far more common.

it's... really never been used in the first way except maybe in jest

the phrase has always meant that you're close like brothers
