Mental breakdowns are serious, but being a complete asshole to other people DOES get you laughed at, and mental illness is not a license to be an asshole.

I work with someone who has delusions of grandeur (also delusions of persecution, etc), and I laugh at them - the delusions, not the person. It's either that, or take them personally because I'm a common target of the delusions, and this person has already burned a lot of bridges with healthcare workers across the city and I'm afraid that if I left, my replacement won't have the same level of endurance or tolerance.

I wouldn't work with Charley though.

Obviously people need to be sensitive to mental health issues but the damage you cause is still the damage you cause and you don't get to not be held accountable for it.
