Ah, ok. See I figured you could still do it (yes you do get one, it's non-missable, toward the end). I myself went back to the clinic through the back entrance waaaay too early and I didn't go to the lower part of the Cathedral Ward until everyone stops talking to you either. Alas, there was no way I could get the good ending, even though I wanted to, so perhaps I will with NG+

Oh man the boss of that area grossed me out so bad. (He more or less identical to one of the bosses in Demon's Souls in how you beat him, though, so I didn't have much trouble. I don't think most people will once they figure him out.)

I might just do that too. I did that in Dark Souls (switched from pure melee to a spell-caster) and it was really rewarding. It was like playing a different game entirely. Idk if that'll really pan out in Bloodborne though, because it's more like different specializations than different classes.
