If you live in a country where employers pay servers a fair wage, then great. But if so, why would you find the idea embarrassing?

The master-servant implications.
Also, the inference that the server, taxi-driver or whatever isn't professional enough to do the job properly without an extra douceur. In a country where such jobs are properly paid, it's insulting.
