I think a good rule of thumb is to always take the lowest possible dosage, and I think it's easy for people to think they need more than that. I actually just double checked my bottle of Advil (ibuprofen) - it does say one is the dosage, though two can be taken if symptoms persist (and you should not exceed six tablets every 24 hours). Yeah, I did mention that doctors will prescribe higher doses, but doctors are also balancing the possible dangerous effects of taking too much with managing pain in their patients. The average person really should avoid taking higher doses without consulting a doctor. If painkiller labels tend to be overcautious, I don't know, that could very well be true, but it still seems advisable to always try the smallest dose first and only take more if you really feel you need it.

Hey, pharmacy person here. It's good that you're trying to minimize the amount of pain medication that you take, but you can take up to 800mg of Ibuprofen three times daily. The OTC ibuprofen is 200mg. Don't suffer in pain just because you think you shouldn't take more. And let me tell you, that doctors who are actually managing pain in patients (to where they see them often for it) are not managing them on ibuprofen. That's where the heavy pain meds come in. Most of they time if you go to see a doctor (without having a chronic pain condition) they're just going to throw a prescription at you and you'll be on your way, no pain management involved.

So it's good that you're being cautious, but it's also unrealistic to think that physicians nowadays are going to be watching you with an eagle eye when you're on any prescription or medication.

Can I ask you what you recommend for cramps when the Doctor won't prescribe something stronger and I have pain so bad that I can barely move and the only thing that even keeps the edge off is taking 800 ibuprofen every six hours? I mean, I don't love taking that much, though I've never had a problem. But naproxen doesn't work for me and my doctor doesn't believe in prescribing anything stronger for "just periods."

There's a medication that a doc I had back east (lovely woman, and when I came back and told her that the rheumatologist she had referred me to said she "didn't treat fibro", and my option was pain management, and told her that I did keep up with research on fibro, so anytime something else came out as a potential treatment, I printed out what I could access from medical journals; she at one point thanked me for it, because it helped her better treat her other fibro patients, when she didn't really have the time -- she'd just had a baby -- to keep up on a specific condition that by rights the rheumatologist should have taken over treatment of) put me on for the debilitating cramps I got on Yasmine/Yaz. Let me see if I can find it. Aha. "Mefenamic acid", aka Ponstel. It's like a super-anti-inflammatory, and should only be taken 3 days out of the month. I usually only got 3 bad days so that was enough for me; I don't know whether that'd be true for you.

Otherwise, there are stronger alternatives, like ketoprofen, which used to be OTC, a relative of ibuprofen. You may have better luck asking about these specific meds vs. "something for pain", which most doctors are going to read as "opiate hunting". Mobic and Voltaren are also stronger options. IMO, nothing worked as well as the Ponstel for cramps, though.
