I've got the perfect solution! I'm a woman in gaming and I've never, ever encountered anything sexist or harrassment-like in gaming, never experienced it as "toxic". Want to know why? I don't do online gaming.
What?! You say, that's victim blaming! Should all women just stay off online gaming?!?
NO!
Devs should just stop focusing on that whole online-crap, and then at least 50% of harrassment would just go away. And without online gaming, games like Uncharted 3 might actually have been decent. Seriously, fuck online gaming.
And then they can focus on the remaining few cases and just ban the assholes, and maybe create two or so more games that maybe cater more to the female demographic to get a bit of balace out there. Five MMOs, all evenly distributed on the scale between "boy's toys" and "girl power", and online gaming only happens there and stops bothering me. wow, that would be the dream.

I'm a woman and I like online gaming. Single-player games bore me, for the most part, and I almost never finish them.

Online gaming is extremely lucrative. They're never gonna stop pumping 'em out.
