It's stated in the game that Heather is Cheryl, the baby given back to Harry in the good end of 1. But otherwise it's a pretty bad game story-wise that just wanted to cash in on the popularity of 1.

It would have been a lot better if Heather were, say, the young daughter of someone who escaped the cult and tried to hide her because she was another viable candidate. In fact, it would have made more SENSE.

I'll have to look it up again, but I don't remember it being that simple. Isn't Heather/Cheryl a split-off part of Alessa's soul or something? Maybe it was less that I didn't understand it and more that I... couldn't accept it? I just thought it was a really stupid idea and, yes, an unnecessary tie back to Harry and Cheryl.
