I don't think it was meant to be ambiguous. Akio is the one who initiates sex with Anthy by grabbing her. After they have sex Anthy speaks to Akio in a flat voice and you can't see her eyes. Since Akio is meant to be Anthy's abuser, how else are you supposed to represent it? Jeez.

except that that interpretation is countered with the fact(? in the flashback) that she first initiated their sexual relationship toward her own end. don't get me wrong, I'm not saying that he's not abusing her, but utena is very lifelike in this respect, and abusive relationships are often ambiguous like that. He feels (and rightly so) that SHE has been the one abusing and manipulating him. In the end, they are both victims of the world they live in (the swords) but while Akio managed to make them work for him and direct them at Anthy, she is perpetually victimized.

what

the world is "mad" because their prince (Akio) isn't perfect, so he makes sure the anger is directed towards the witch (Anthy), so that he can continue being a piece of shit without losing face

she doesn't fight back because that's the role she's supposed to be playing (because her brother is good and she's bad) and she remains pretty much catatonic/despondent until Utena shows up and Anthy realizes it's okay to put herself first/not answer to Akio's needs

the entire point is that Anthy is victim-blamed (even she herself thinks she's the one responsible) and the situation isn't resolved until Anthy leaves, because Akio is perfectly happy with using Anthy as he pleases

it's made perfectly clear she didn't initiate shit

except when she seduces him when they're children, when he's fallen victim to the world's expectations of him?
maybe take a step back and reevaluate your idea of abuse existing within clear good/bad frame, then the whole series will make more sense. You're stereotyping the victim, and that erases part of the story, and doesn't help with solutions of similar situations, nor is it the point Ikuhara wants to make. All of his works deal with the guilt of the victim vs. the (previous) victimization of the abuser. trying to force it into a black/white frame is not doing it justice in the least.
