With Garnet, I just assume her "clothing" is a hard outer shell. Whatever's underneath it is whatever Gems have on the inside (if a hard-light projection has "insides" to speak of.) Amethyst's a bit different in that she clearly models her projected outfit on human clothing, but it's still part of her in some way, since it disappears if she's hurt and retreats into her gem.
