I can't say anything about self-harm specifically, as that's not something I've ever had to deal with myself, but "help" from someone is a very broad concept. Your usual "love fixes it!!" situations for serious, hard psychological issues are bullshit because there is no internal work being done, nothing changes within the character. Psychological problems are, by their nature, inside problems (outside triggers of these problems are a different matter) and must be solved from inside.

Someone knowing about an issue may not help, but someone knowing AND helping you to work on the issue by providing support, suggesting coping techniques, or showing you the possible roots of the issue in question may make things significantly better.
