John on Farscape due to trauma and repeated alien mindfucks, with the most drastic being the mind-control biotech chip. The worst of it goes away after the chip does, but he is still obviously not as stable as he once was.

Also on Farscape, Crais goes... maybe not insane, but he certainly becomes irrational over his brother dying and Crichton escaping.

And of course there are regular mindfucks to send people over the edge for an episode or two (e.g. "Crackers Don't Matter"), but I'm not sure that counts.
