I don't think I ever had any given idea of what it meant (barring that one time when I was a kid that I made a collage that included a horse eating a pan of fried potatoes and joked that you don't look a [gift] horse in the mouth or you'll see it's dinner), but it was a long time before I connected it to the age-checking thing.
