I'd wager 45% is pure fetish and 50% is reaching for sjw brownie points.

But in the case of sexual fanfic/fanart with trans "representation", I'd wager 99.999% is pure fetish.

I'd say at least 5% is "They're not a boring heterosexual white couple! He's FMAB agender demisexual who uses he/him and she's trans but due to sciencemagic she's indistinguishable from a cis woman."

Yep. Anytime I see NSFW art with trans characters, it's always just focusing on their genitals and fetishizing trans people in general.
