It's pretty easy, actually.

I (and my partner at the time) was fortunate enough that I was triggered during foreplay and just about climbed the wall to get away, but I've spoken to other survivors who've gotten a lot further before they hit that wall, or for whom it's been something specific to a given encounter (their partner tried dirty talking, or pinned their arms while shifting position, or any of a thousand other things) when other instances of sex have gone just fine. It's not a value judgment on any of our partners - they did nothing wrong - but being catapulted back into that headspace because something, be it external or just our own reactions, really does feel like reliving rape, and unfortunately having been the one to initiate doesn't provide any protection from that.
