At least in fandom and your imagination usually no people are actually hurt or abused in the process (which let's be honest isn't always the case in porn/rl).
I think this escalation of what turns a person on is sort of par for the course in anything erotic, isn't it? That's why people go for more and more intense/weird/kinky stuff.
