First, you are awesome for taking in an elderly homeless cat, that is so cool of you to do.

I mean, I guess he could just not know how to use litterboxes if he is going outside? A litter box should be "natural" for a cat, but a rug or bed might also feel "natural" to them. I haven't personally had the problem, but tips I've heard before include: scoop his poop up, wherever he goes, and put it in the litter box. Possibly move the box or add another one, could be he doesn't like the location. You could temporarily put the box in the kitchen or hall, just so he gets used to using it, then gradually move it to the spot you want. You could also maybe determine or try to move out some issue out of his control - vet says he is physically healthy so maybe unexpected bowel movements aren't an issue for him, could he be going in certain spots out of anxiety?

I was dreading putting a box in the kitchen but maybe you're right because that's the spot he's decided on. Definitely no unexpected bowel movements (he'll sometimes circle around the house, waiting for me to leave the kitchen if he has designs on it).
