Not if they are still raised together. I don't know the fandom, but often these sorts of AUs will have the characters still raised together, just one of them adopted.

Yeah, usually they are raised together, and quite often they are also never told that one is adopted while the other isn't. Though that depends on the fandom and how hard yaoi tropes begin to figure in.

drives me bonkers that people don't view that as incest because ones adopted. still family. would still have all the issues. genetics ain't the main problem here.

Well, different people feel different ways on this one. Personally, when it comes to sibling or cousin incest ships, genetics ARE my main issue with it.
