Depends on why you fall off the wagon. I've self-sabotaged several times in the past, and I don't always really understand the reasons, although it can ironically be fear of failure that's causing it, ergo it's not failure if I just give up and stop trying. You have to really sit and think about what you want, why you want it, and why you're not doing it if you really want it.

All of that being said, one of the tools I used to try to help myself stay on track was an online account at beeminder. You can sign up for free, and you input a goal - in my case, it was weightloss. Every day, it emails you for an update, and as long as you stay on track, it's free. If you go off track once, you get a free do-over. However, if you fail again, you have to pay to start over. If you fail again, you have to pay more to start over. The theory is to find a point of pain for you so that you commit and don't completely fall off the wagon.

It wasn't the only thing that worked for me, but it was a helpful tool to keep me on track.
