I don't think anyone's arguing that there weren't any backward medical practices in that time period, only that Watson isn't portrayed as giving people arsenic (or leeching anyone) in the actual canon, so there's no need to retroactively edit out any mistakes that stem from the fact that he's a product of his time.

To sum up: You do necessarily not have to sacrifice historic accuracy in order to portray Watson as a good doctor, even by modern standards.
