I agree with your whole comment, and I think this is a really important point. Not to mention, there's a strong element of "toxic bureaucracy" in there too, and the way in which our bureaucratic systems for dealing with the mentally ill are often incompetent to the point of cruelty, traumatic for the mentally ill, and only succeed in exacerbating their unwellness.

I think In Cuckoo's Nest we get a pretty good picture of what often happens when a system for dealing with disempowered individuals discourages genuine compassion and helpfulness in its employees. A system which prioritizes minimizing the effort and cost expended, with almost complete disregard for the human cost of doing so.
