I know for a fact that Peter Moffatt (who directed Frazer Hines and Patrick Troughton in The Two Doctors) said that they were milking the script for every double entendre that they could. And also, apparently it was some sort of inside joke between the two of them to inject the following phrase into every episode they could:

Jamie: Would you look at the size of that, Doctor!
Doctor: Yes, Jamie, that is a big one, isn't it?

Even when the script didn't support it!

49--I think he would too. I can't see him cutting Phoenix dead and walking away, no matter what he'd done. Smacking some sense into him and dragging him off to get some decent clothes, maybe.
