It's been over a decade since I read that book, but IIRC, Idgie courts Ruth, Ruth's initially afraid of their relationship because she's a religious type and runs away, she runs away from her abusive asshole beard husband quoting the "Whither thou goest, I will go" scripture, Idgie's father has a "man-to-man" talk with Idgie about doing right by her woman, they raise a child together, and when Idgie and Ruth fight, Idgie winds up kicked out of their shared bedroom. So... yeah, the sexual relationship is implied in the same way that most couples that you know would be, in that you haven't actually seen them have sex.
