1) As other people have mentioned, that wasn't Dean, it was a hallucination Sam was having.

2) While the way they tried to get him clean was the exact opposite of ideal, there does have to be some recognition of the fact that Sam wasn't exactly addicted to a normal drug. He was addicted to demon blood, which gave him superpowers when he was taking it and made him progressively more and more evil, and detoxing made him telekinetically throw things, levitate, and fly around the room flinging himself into the walls when he wasn't strapped down. They couldn't check him into rehab. It wasn't safe for them to be in the room with him. It wasn't safe for him to be unrestrained. There weren't really any good options for dealing with that shit.

A lot of the times, they really don't HAVE any good options. They've got the option that really sucks a whole lot, and the option that maybe sucks a little less.
