Dean asked Death to get him out when he retrieved Sam's soul, Death said Dean could have to choose, Dean chose to get Sam's soul out.

Though from what I heard, one of TPTB said that when Castiel molotoved him, that his body was destroyed and Adam got to go to heaven, just like Jimmy did when Cas died the first time.

AYRT, thanks for update! I always felt bad for poor Adam, who was used as a deus ex machina, and then just put aside and entirely forgotten. Despite the plight of being, you know, in hell. And as for TPTB, how hard would it have been to include an expository line indicating that he'd gone to Heaven? Not hard at all, which means it all really just was an afterthought. Nevertheless, I'll accept the retroactive explanation, and the late-breaking moment of concern from Dean. :)
