I dislike it when people try to apply modern Western norms to other times and places. For FY, it's two-fold: the series being set in historical China, and the series being made in 90s Japan. Both places and time periods had much more conservative views towards gender and biological sex. Maybe the series would have been written differently in this more accepting time. To me, I interpreted the situation as Nuriko presenting as female because it was the only way to get romantically close to Hotohori, not because he thought he was born the wrong sex.

Nuriko started presenting as female because their little sister died and they wanted to take her place (using gender neutral pronouns here just because). They had been living as a woman well before ever being recruited to the imperial harem.

Nuriko's gender is a whole clusterfuck of ignorance and stereotyping, ultimately resulting in a pretty cringey scenario. People may not have known better then, but that doesn't stop it from being pretty uncomfortable in retrospect.

No, they didn't. In comparison to the Western world, Asian societies had very lax views on gender in terms of history before colonization an evangelism. Even though the SJW notion of "whites invented homophobia!" is a lie, acting like every single place in the world was anti-homophobic or anti-trans is a pop culture myth. Yeah 90s Japan might not be that way since Japan isn't majority gay friendly, but historic China? Nothing at all like the modern police state that it is today.
