Humans are hardwired to see certain things a certain way, like faces for instance since we're on the topic of looks. If anything deviates from what your brain is programmed to recognize as "normal", people tend to get uncomfortable or scared. It's not bigotry/phobia to have that reaction. It falls into bigotry/phobia when people CONTINUE acting uncomfortable or even start demonizing said handicapped person like you said, long after the instinctive reaction went away.
