But I think maybe the whole chibi thing is about getting the "comfort/care" part of hurt/comfort without the hurt. Y character is now small and vulnerable and needs cared for! (without being in a bodycast or braindead or whatever).

Pretty much that.

I've never really seen the squick-inducing stuff that op mentions because for the most part, this is a subgenre of hurt/comfort but with de-aging as the theme. Heck, all the stories I've seen have had the de-aging played for drama's sake, with the de-aged character still an adult inside, plagued with represseed childhood memories resurfacing, and also having to be dependent on another person for reaching stuff high up, etc.
