I'd say when I see it in non-fandom context it's almost always used correctly.

When I see it in fandom, it's almost always when they mean squick. For example, as part of their art show Star Wars Celebration used a fanart piece that could be see as Rey/Kylo. Cue the people screaming that it should be warned for as a trigger.
